Estimating disease prevalence using a tst of unknown sensitivity and specificity, as defined epidemiologically, may lead to gross errors. Thus, sensitivity and specificity are defined and methods for determining them are discussed. The effects of sensitivity, specificity and the unknown true prevalence of disease on apparent prevalence and predictive values are described. If tests have a good sensitivity (<80%) and specifity (>90%), then the apparent prevalence, will be a resonable estimate of true prevalence, and the predictive value of a positive test will be at least 70% if the true prevalence of disease is at least 10%. |
